MEMBER TERMS OF SERVICE
Juma Group LLC DBA Jumavo, a Delaware limited liability company
3851 Katella Avenue, Suite 225
Los Alamitos, California 90720
Effective Date: June 1, 2026
This summary is provided for your convenience. It is not legally binding. The full legal terms below govern your use of the Jumavo Wallet and Membership Platform. By using Jumavo, you agree to these Terms of Service.
In plain English: Jumavo is a prepaid wallet that lets you fund and use cash-pay healthcare across many providers, all in one place. You subscribe monthly, your dollars become Jumavo Credits, and you spend those Credits at any provider in the Jumavo Network. Credits never expire, but they are not cash. You stay in charge of your account and can cancel anytime.
The Most Important Things to Know
  • Jumavo is a prepaid wallet and membership platform. Your monthly subscription converts into Jumavo Credits that you spend across participating cash-pay healthcare providers in the Jumavo Network.
  • Jumavo Credits never expire and are non-cash. You cannot transfer them to another person, exchange them for cash, or use them for services covered by insurance, Medicare, Medicaid, HSAs, or FSAs.
  • Your subscription auto-renews monthly. You can cancel anytime through the Jumavo app, and the cancellation takes effect at the end of your current billing cycle. We do not refund prior periods.
  • Jumavo is not a healthcare provider. We do not practice medicine or dentistry. All clinical care comes from the licensed independent providers in our network. We are not responsible for the quality of clinical care.
  • Disputes between you and Jumavo are resolved by binding arbitration in Los Angeles through JAMS, with a 30-day opt-out window after you sign up. Class action lawsuits are waived.
  • Your Provider, not Jumavo, is the HIPAA Covered Entity for your healthcare. Jumavo operates as a HIPAA Business Associate of your Provider under a signed Business Associate Agreement. We safeguard your health information using encryption, access controls, audit logging, and HIPAA-eligible infrastructure.
  • This Agreement is governed by California law. We honor California Consumer Privacy Act and California Privacy Rights Act (CCPA/CPRA) rights, the California Automatic Renewal Law (ARL), and California Civil Code §1749.5 (Credit non-expiration).
PLEASE READ THESE TERMS OF SERVICE CAREFULLY. BY ACCEPTING THESE TERMS, REGISTERING FOR A Jumavo ACCOUNT, OR USING THE Jumavo SERVICE, YOU AGREE TO BE BOUND BY THESE TERMS, INCLUDING THE MANDATORY ARBITRATION AND CLASS ACTION WAIVER PROVISIONS IN SECTION 18. IF YOU DO NOT AGREE TO THESE TERMS, DO NOT USE THE Jumavo SERVICE.

1. Acceptance of Terms

1.1 Acceptance. By clicking "I Agree," creating a Jumavo account, funding a Jumavo Wallet, or otherwise accessing or using the Service, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy, which is incorporated by reference.

1.2 Eligibility. You must be at least eighteen (18) years of age, a resident of the United States, and capable of entering into a binding contract to use the Service. The Service is not available to minors. Use of the Service by anyone under eighteen (18) is prohibited.

1.3 Updates to Terms. Jumavo may modify these Terms from time to time. Material changes will be communicated to you through the Service or by email at least thirty (30) days before they take effect. Your continued use of the Service after the effective date of any update constitutes your acceptance of the updated Terms.

2. The Jumavo Service

2.1 What Jumavo Is. Jumavo is a consumer wallet and membership platform that allows Members to prepay for cash-pay healthcare services through a single, consolidated wallet. Members fund their Jumavo Wallet through a monthly subscription. Funds are converted into Jumavo Credits, which Members can redeem for Cash-Pay Services at Participating Providers within the Jumavo Network.

2.2 What Jumavo Is Not. Jumavo is not a healthcare provider, insurance company, health plan, money transmitter, bank, or stored-value card issuer. Jumavo does not practice medicine, dentistry, or any healthcare profession. Jumavo does not provide clinical advice, diagnosis, or treatment recommendations. All clinical care is provided by Participating Providers, who are independent licensed healthcare professionals.

2.3 No Insurance Substitution. Jumavo is not insurance. Jumavo does not pay for, reimburse, or cover any portion of services that are or would be covered by your insurance, Medicare, Medicaid, HSA, FSA, or any other third-party payor. Jumavo Credits may not be used to pay for insurance copays, deductibles, or coinsurance.

3. Account Registration and Security

3.1 Registration. To use the Service, you must register a Jumavo account by providing accurate, current, and complete information about yourself, including your name, email address, billing information, and identity verification information as required by applicable law.

3.2 Identity Verification. Jumavo may take reasonable steps to verify your identity as required by applicable law, using the information you provide.

3.3 Account Security. You are responsible for maintaining the confidentiality of your account credentials. You agree not to share your account credentials with any third party. You are responsible for all activity that occurs under your account. You must notify Jumavo immediately at [email protected] if you believe your account has been compromised.

3.4 One Account Per Person. Each Member may maintain only one (1) active Jumavo account. Duplicate or fraudulent accounts may be terminated without notice.

4. Membership Tiers and Subscriptions

4.1 Membership Tiers. Jumavo offers the following subscription tiers, each of which may be modified by Jumavo from time to time with notice:

(a) Member, at ninety-nine U.S. dollars ($99.00) per month;

(b) Ambassador, at one hundred ninety-nine U.S. dollars ($199.00) per month; and

(c) Executive, at two hundred ninety-nine U.S. dollars ($299.00) per month.

4.2 Conversion to Credits. Each monthly subscription payment is converted into Jumavo Credits in your Jumavo Wallet at a rate of one U.S. dollar to one (1) Credit. Conversion occurs automatically upon successful processing of the subscription payment.

4.3 Recurring Billing Authorization. By subscribing, you authorize Jumavo to charge your payment method on a recurring monthly basis at the subscription tier you have selected. Charges occur on the same calendar day each month, beginning on the day you initially subscribe. You are responsible for ensuring that your payment method remains valid and has sufficient funds.

4.4 Tier Changes. You may upgrade or downgrade your subscription tier at any time through the Service. Changes take effect at the start of your next billing cycle. Jumavo does not prorate subscription charges for tier changes within a billing cycle.

5. How Jumavo Credits Work

5.1 Closed-Loop Credits. Jumavo Credits are closed-loop digital credit units redeemable solely for Cash-Pay Services at Participating Providers within the Jumavo Network. Credits are not cash, not legal tender, and not redeemable for cash, except as required by California Civil Code §1749.5(b)(2) for remaining balances under ten U.S. dollars ($10.00).

5.2 No Expiration. Pursuant to California Civil Code §1749.5(a), Jumavo Credits do not expire. Credits remain in your Jumavo Wallet and available for redemption regardless of the length of time they have been held.

5.3 Non-Transferable. Jumavo Credits are non-transferable. Credits may be redeemed only by the Member whose Jumavo Wallet holds them. You may not transfer, gift, sell, trade, or assign Jumavo Credits to any other person or entity.

5.4 No Cash Value. Jumavo Credits do not constitute cash, stored value of money, or a money transmission instrument. Jumavo Credits represent your right to receive Cash-Pay Services from Participating Providers in the Jumavo Network.

5.5 Rewards and Boosts. Jumavo may, from time to time, offer rewards, bonuses, promotional credits, or Boosts to Members. Boosts may be funded by Participating Providers and offered to Members at their discretion. All rewards and Boosts are subject to the terms of the specific promotion, are non-cash, non-transferable, and may be modified or revoked by Jumavo or the funding Provider in accordance with the promotion terms.

6. Redemption of Credits

6.1 Where to Redeem. You may redeem Jumavo Credits only at Participating Providers within the Jumavo Network. A current list of Participating Providers is available through the Service.

6.2 How to Redeem. To redeem Credits, present your Jumavo account at the time of payment at a Participating Provider. The Participating Provider will process the redemption through the Jumavo Platform, debiting your Wallet for the Transaction Value of the Cash-Pay Services.

6.3 Provider Acceptance. Jumavo does not guarantee that any specific Participating Provider will be available, accept your Credits at any specific time, or offer any specific service. Participating Providers may withdraw from the Jumavo Network at any time in accordance with their agreement with Jumavo.

6.4 Cash-Pay Services Only. Jumavo Credits may be redeemed only for Cash-Pay Services. You may not use Jumavo Credits to pay for any service that is or will be billed to or reimbursed by insurance, Medicare, Medicaid, HSA, FSA, or any other third-party payor.

6.5 Service Quality. The Participating Provider, not Jumavo, is responsible for the quality and outcome of the Cash-Pay Services delivered. Disputes about the quality of care, treatment outcomes, or provider conduct must be addressed directly with the Participating Provider.

7. Refunds and Cancellations

7.1 Subscription Refunds. Jumavo does not refund subscription payments for periods that have already begun. If you cancel your subscription, you retain access to the Service and your existing Credits until the end of your current billing cycle.

7.2 Service Refunds. If a Participating Provider issues you a refund for a Cash-Pay Service, the refund will be returned to your Jumavo Wallet as Credits in the amount of the refund, in accordance with the Participating Provider's refund policy.

7.3 Erroneous Charges. If Jumavo charges you in error (for example, a duplicate charge, an unauthorized transaction, or a billing error), notify Jumavo at [email protected] within sixty (60) days of the erroneous charge, and Jumavo will investigate and, if appropriate, refund the erroneous amount.

7.4 Cancellation. You may cancel your subscription at any time through the Jumavo Service. Cancellation takes effect at the end of your current billing cycle. After cancellation, you may still redeem any remaining Credits in your Jumavo Wallet for Cash-Pay Services at Participating Providers. Credits remain valid in accordance with Section 5.2.

8. Automatic Renewal Disclosure (California ARL)

CALIFORNIA RESIDENTS: This Section 8 contains important disclosures required by the California Automatic Renewal Law. Please read carefully.
8.1 Automatic Renewal. Your Jumavo subscription is an automatically renewing subscription. Your subscription will renew automatically each month at the subscription tier you have selected, and your payment method will be charged accordingly, until you cancel.

8.2 Subscription Charges. The amount you will be charged each month is the subscription tier price you selected at signup: $99.00, $199.00, or $299.00 per month, plus any applicable taxes. If Jumavo increases the subscription price, Jumavo will notify you in writing at least thirty (30) days before the change takes effect.

8.3 How to Cancel. You may cancel your subscription at any time through the Service by navigating to your account settings and selecting "Cancel Subscription." You may also cancel by contacting Jumavo at [email protected]. Cancellation is effective at the end of your current billing cycle.

8.4 Acknowledgment. By subscribing, you affirmatively acknowledge and agree to the automatic renewal, the recurring charges to your payment method, and the cancellation method described above.

9. Provider Services and Independent Providers

9.1 Independent Providers. All Cash-Pay Services are provided by Participating Providers, who are independent licensed healthcare professionals. Jumavo does not employ Participating Providers, does not control their clinical decisions, and does not supervise their practice.

9.2 No Provider Endorsement. Jumavo does not endorse, recommend, or guarantee the quality of any Participating Provider's services. Listing in the Jumavo Network does not constitute an endorsement by Jumavo of the Provider's qualifications or services.

9.3 Your Responsibility. You are responsible for selecting your Participating Provider, verifying their credentials, and making informed decisions about your healthcare. Jumavo encourages you to verify any Participating Provider's licensure and credentials before receiving services.

9.4 No Liability for Provider Acts. Jumavo is not liable for any act, omission, error, malpractice, or other conduct of any Participating Provider, including but not limited to misdiagnosis, treatment errors, billing errors, breach of patient confidentiality, or any other claim arising from the Cash-Pay Services.

10. Privacy and Data Use

10.1 Privacy Policy. Jumavo's collection, use, disclosure, and protection of your personal information are governed by our Privacy Policy, available at jumavo.com/privacy, which is incorporated into these Terms by reference.

10.2 California Consumer Privacy Act. If you are a California resident, you have specific rights under the California Consumer Privacy Act and California Privacy Rights Act, including:
  • the right to know what personal information Jumavo collects about you;
  • the right to delete personal information;
  • the right to correct inaccurate personal information;
  • the right to opt out of the sale or sharing of personal information; and
  • the right to limit the use and disclosure of sensitive personal information.
To exercise these rights, contact Jumavo at [email protected] or visit jumavo.com/privacy.

10.3 Communications. By providing Jumavo with your email address and phone number, you consent to receive transactional, account-related, and service-related communications from Jumavo via email, SMS, and push notification. You may opt out of marketing communications at any time. Transactional and account-security communications are necessary for the Service and cannot be opted out of while you maintain an active account.

10.4 HIPAA and Protected Health Information. You acknowledge and understand the following:

10.4.1 When you receive care from a Participating Provider in the Jumavo Network, certain information about that care (including diagnostic codes, treatment categories, dates of service, and provider identity) may be transmitted through the Jumavo Platform as Protected Health Information (PHI) under HIPAA.

10.4.2 Jumavo serves as a Business Associate of your Participating Provider under HIPAA. Jumavo has entered into a Business Associate Agreement with each Participating Provider that requires Jumavo to safeguard PHI in accordance with the HIPAA Privacy Rule, Security Rule, and Breach Notification Rule.

10.4.3 Jumavo uses and discloses your PHI only as permitted by HIPAA and the Business Associate Agreement with your Provider, including for the purpose of:
  • operating the Jumavo Platform;
  • processing Transactions and Settlement;
  • routing referrals;
  • providing customer support; and
  • performing analytics on behalf of Providers.
Jumavo does not sell PHI or use PHI for marketing without your authorization.

10.4.4 Jumavo implements administrative, physical, and technical safeguards reasonably designed to protect PHI, including encryption at rest and in transit, role-based access controls, audit logging, multi-factor authentication, workforce HIPAA training, and operating on HIPAA-eligible cloud infrastructure.

10.4.5 Jumavo's subcontractors that process PHI are required to enter into HIPAA-compliant Business Associate Agreements with Jumavo before receiving any PHI.

10.4.6 Notice of Privacy Practices for your healthcare is provided by your Participating Provider, who is the Covered Entity under HIPAA. To exercise HIPAA rights regarding your medical records (including the right of access, amendment, accounting of disclosures, and restriction), please contact your Participating Provider directly.

10.4.7 If you believe your PHI has been used or disclosed in violation of HIPAA, you may file a complaint with your Participating Provider, with Jumavo at [email protected], or with the U.S. Department of Health and Human Services Office for Civil Rights.

12. Intellectual Property

12.1 Jumavo Ownership. The Service, including all software, text, images, graphics, logos, trademarks, and other content (excluding User Content), is owned by Jumavo or its licensors and is protected by U.S. and international copyright, trademark, and other intellectual property laws.

12.2 Limited License. Jumavo grants you a limited, non-exclusive, non-transferable, revocable license to access and use the Service for your personal, non-commercial use, subject to these Terms.

14. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, JUMAVO AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AND AFFILIATES SHALL NOT BE LIABLE TO YOU FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES (INCLUDING LOST PROFITS, LOST DATA, OR LOSS OF USE) ARISING OUT OF OR RELATING TO THE SERVICE, EVEN IF JUMAVO HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
14.1 Cap on Liability. Jumavo's total cumulative liability to you under these Terms, whether arising in contract, tort, or otherwise, shall not exceed the greater of: (a) the total amount of subscription fees you have paid to Jumavo in the twelve (12) months preceding the event giving rise to liability; or (b) one hundred U.S. dollars (US$100.00).

14.2 Exceptions. The limitations in this Section 14 do not apply to: (a) Jumavo's indemnification obligations; (b) Jumavo's gross negligence or intentional misconduct; or (c) any liability that cannot be limited under applicable law.

14.3 California Civil Code §1542 Waiver. If you are a California resident, you expressly waive the protections of California Civil Code §1542, which provides: "A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party."

15. Indemnification

15.1 Your Indemnification. 16. Termination

16.1 Termination by You. You may terminate your account at any time by following the cancellation instructions in Section 7.4.

17. Governing Law

17.1 Governing Law. These Terms are governed by the laws of the State of California, without regard to its conflict-of-laws provisions.

18. Mandatory Arbitration and Class Action Waiver

PLEASE READ THIS SECTION CAREFULLY. IT REQUIRES YOU TO RESOLVE DISPUTES THROUGH BINDING ARBITRATION RATHER THAN COURT. IT INCLUDES A CLASS ACTION WAIVER. YOU HAVE A RIGHT TO OPT OUT OF THIS ARBITRATION PROVISION WITHIN THIRTY (30) DAYS OF FIRST ACCEPTING THESE TERMS, BY FOLLOWING THE INSTRUCTIONS IN SECTION 18.4.
18.1 Informal Resolution. Before initiating arbitration, you agree to attempt to resolve any dispute by contacting Jumavo at [email protected] and describing the dispute in writing. Jumavo and you agree to attempt in good faith to resolve the dispute through informal discussion for at least thirty (30) days before initiating arbitration.

18.2 Binding Arbitration. Any dispute, claim, or controversy arising out of or relating to these Terms, the Service, or your relationship with Jumavo (each, a "Dispute") that is not resolved through informal discussion shall be resolved by final and binding arbitration administered by JAMS in Los Angeles, California, pursuant to the JAMS Streamlined Arbitration Rules and Procedures then in effect. The arbitration shall be conducted by one (1) neutral arbitrator. The arbitrator's decision shall be final and binding, and judgment on the award may be entered in any court of competent jurisdiction.

18.3 Class Action Waiver. TO THE FULLEST EXTENT PERMITTED BY LAW, YOU AND JUMAVO EACH WAIVE THE RIGHT TO PARTICIPATE IN A CLASS ACTION, CLASS ARBITRATION, OR ANY REPRESENTATIVE PROCEEDING. ARBITRATION SHALL BE CONDUCTED ON AN INDIVIDUAL BASIS ONLY, AND THE ARBITRATOR SHALL HAVE NO AUTHORITY TO CONSOLIDATE CLAIMS, CERTIFY A CLASS, OR PRESIDE OVER ANY FORM OF CLASS OR REPRESENTATIVE PROCEEDING.

18.4 Opt-Out Right. You have the right to opt out of the arbitration and class action waiver provisions of this Section 18. To opt out, send a written opt-out notice to Jumavo by email at [email protected] within thirty (30) days of first accepting these Terms. The opt-out notice must include your name, the email address associated with your Jumavo account, the date you first accepted these Terms, and a clear statement that you wish to opt out of the arbitration provision. If you timely opt out, disputes shall be resolved in the state or federal courts located in Los Angeles County, California.

18.5 McGill Carve-Out. Notwithstanding anything to the contrary in this Section 18, claims for public injunctive relief brought under the California Consumer Legal Remedies Act, California Unfair Competition Law, or California False Advertising Law may be brought in court and are not subject to arbitration, in accordance with California law as articulated in McGill v. Citibank, N.A.

18.6 Exceptions. Notwithstanding the foregoing, either party may seek temporary or preliminary injunctive relief in a court of competent jurisdiction to protect intellectual property rights, confidential information, or to prevent imminent harm. Such application shall not be deemed a waiver of arbitration.

18.7 Costs of Arbitration. Jumavo will pay all arbitration filing fees, administrative costs, and arbitrator fees that Jumavo is required to pay under JAMS's consumer rules. If you are required to pay any arbitration fees that would not be incurred in court, Jumavo will reimburse you for those fees, except where the arbitrator determines that your claim is frivolous.
